Let me see if I remember the steps correctly.

Opening the file should put it in your Itunes library under tones. Plug in your iphone and sync tones.

Then disconnect and go to settings, then sounds, then ringtones if you want it for your ringtone or text tones, new voicemail tone, etc.. Scroll down and select the one you want. Should work.

Hah! I used to have that on my old phone - it'd play whenever someone left a voicemail. Now, on my iPhone, I have that "duhn-duhduhn~strummm" suspense sound from Pandora. My ringtone for the past several years has been the "blip...blip...blip" from the "hatch" in Lost. I love it, but it sucks whenever I'm near the checkout line at grocery stores...I always think my phone is ringing
